To
keep our fees to a minimum, payment for services is asked for at
the time of your office visit. If you cannot pay immediately,
a payment plan can be arranged for you.
Please bring your health
insurance card and any insurance forms with you to your office
visit. Out staff will be happy to assist you with answers
to questions regarding your coverage. If your insurance
requires referral authorization, you must have the appropriate
form for each visit or that visit will have to be rescheduled.
If you are covered under Worker's
Compensation, you must bring a written authorization letter to
your first visit or it will have to be rescheduled. All
testing will be authorized as needed.
Since seating is limited, we ask
you to limit children, friends and relatives accompanying you to
the office. We realize, however, that a babysitter and
transportation problems do arise that give you no choice in the
matter.

Approximately
a week before your appointment, you will receive a questionnaire
from us in the mail. It asks for information about
yourself that is necessary to provide you with the best possible
care and service. Please fill it out and bring it with you
to your appointment.
State
and Federal laws strictly control prescription
medications. To give our patients the highest quality
care, medications are prescribed at office visits only.
You will be given enough
medication to last until your next office visit. Please
take medication as directed so it will last the prescribed time
period.
